Health care reform and doctors challenges



Healthcare changes are coming, whether Doctors like it or not! Obamacare is here and the changes are just beginning. These changes are going to be effecting everyone in this country. Like it or not, healthcare reform is here. One thing is for show, things are going to change! Doctors in this country have been seeing their incomes drop and paperwork going up. I am always an optimist, however, I have never seen the government get involved in anything and cause less paperwork and more money. I can be assured that there will be more paperwork and probably a little less money for doctors.
Doctors have seen decreasing incomes over the past few years, even longer. The so-called Mercedes 80’s are long long gone! I wasn’t even ever around to experience it. Well, what does this mean. No one really knows what healthcare reform will bring. Healthcare reform will add many new patients to the market, making more patients available to the average doctor. The question will be, how will the reimbursement be, and how much work will it be.  In my experience with certain insurances run by states or federal government, pay tends to be much lower and paperwork much higher. I stopped accepting Medicaid in my office years ago because it was costing me more to file claims and do paperwork than it was paying!

This type of scenario will probably be pretty likely in the future. Some of the new federal rules will require more paperwork and less reimbursement. It will be a difficult decision to decide which ones to accept and which to not. Doctors assume they don’t have a particular cost with each patient, but they are wrong. Every patient costs money, whether it is just doctor and staff time, wear and tear on your equipment, and many procedures have fixed costs, supplies, medications, etc. It is not free to treat a patient. You should try to figure out your costs per patient visit, this is not always possible, but it can be. Figure it out, or guestimate the best you can.
So, what are doctors going to do. Things will change and you better change as well. The paperwork will be burdensome, and you better figure it out. If it matches the trends we are seeing now, there will be more. The big thing with these forms, paperwork, etc, is that someone probably already did it. Why re-invent the wheel? The appeal letter you need, office form you need, letter of discipline you need, someone probably already wrote it! Why would you spend hours fixing it yourself? There are two things you are allowed to do in your practice. 1. Treat Patients. 2. Work on your business. Don’t do anything else. 

I can say this, of course, because I was a terrible offender. I would spend days working on a PowerPoint for presentation, writing an appeal letter, staff disciplinary letter, something. I spent many hours on this, and I did it well. However, it was not a great way to spend time. But, since I was good at it, everyone always asked me for my paperwork. I didn’t mind helping friends, but I figured there had to be another way. I couldn’t find anything, so I decided I would have to make something. So, I had a web developer make me site that would allow docs to buy, sell, and trade paperwork, spreadsheets, PowerPoint’s, just about anything you would need. Chronic Back Pain By Dr. James R. Fedich, Chiropractic Physician


New research has show that the pain in your back can also be a pain in your brain.  Research conducted at Northwestern University has discovered that chronic lower back pain can deteriorate brain function.  The study was performed on 42 individuals, half with chronic back pain and half without.  The study found that in patients with six months or longer of low back pain they had lost about 1.3 cubic centimeters of brain matter. 

This much loss of brain matter is equal to the amount lost in ten to twenty years of aging.  The study also found that every year a person has chronic low back pain, they lose the same 1.3 cubic centimeters of brain matter.  This essentially means that for every one year that you have chronic back pain, your brain ages 10 – 20 years.  So, if you have lower back pain for 3 years, which many people have, your brain may have aged 60 years in that time.  This is some very significant research.

 Essentially, that achy low back you have been putting off for years has been slowly destroying your brain.  Dr. A. Vania Apkararian who headed the study states that six months or longer of lower back pain is accompanied by abnormal brain chemistry, indicated by chemical changes in the brain that can affect emotional assessments, decision making, and control social behavior.  Dr. Apkararian further states, “Chronic pain is a state of continuous persistent perception with associated negative affect and stress.”  


 This research supports what chiropractors have been saying for years.  Your body is a highly tuned machine, and when a part of that machinery is not working properly, it disrupts the rest of your system.  If you have nerve compression in your low back, and the impulses from your brain are not allowed to travel freely down the nerves, then of course this will affect your brain.

 Your brain is always monitoring the body, and if it cannot send and receive messages to a part of the body, this interferes with its system.  The nervous system is a highly tuned machine, and just like your car it needs a tune up.  Research has now confirmed that the achy low back that you have not worried about for years is slowly destroying your brain.  Now is as good a time as any to get a check up and make sure that everything is working well.  Nobody ever looks back on life and wishes they had taken worse care of their body.  Take care of yourself now before you regret it later!

What does Health Reform mean to you?

National Health Reform is a law now, and what exactly that will mean for all of us, doctors and patients, is yet to be seen. One thing for sure is that things will change.


I do agree that insurance companies have gotten out of control and are too often dictating peoples health care choices. The Health insurance companies started the trend of people giving control of their health to someone else. Originally, people were responsible for their own health. They took care of themselves best they could, and when things went wrong, they paid a doctor to fix them. Then, health insurance companies came around and took over that responsibility for patients. Patients could do whatever they wanted, not worry much about their health and the health insurance companies would pick up the tab. That model stayed around for a while, until the health insurance companies realized it was getting out of control expensive. So, instead of trying to get people to assume the responsibility for their health, they now cut services, cut doctors fees, and limit care. Well, after about fifteen years of this, we finally got fed up, doctors and patients.

So, good old uncle sam has come in to save the day. So, with our new plan of heath care coming down the works, we have decided to pass along the responsibility of our health to the government. We went from controlling our own healht, to letting insurance companies control our health, to now letting uncle sam control our health. I dont know about you, but I dont see too many things the government runs too well. I am certainly not going to entrust them with my health. NO WAY!

So, where does this leave us? The details of the heath reform bill will take years to come out, and the effects we wont know for a while. One thing is for certain, we traded one giant corrupt organizaiton for another larger, more corrupt one. This is the time when we need to take back the responsibility for our health. Who will pay when a major issues strikes is important, but we cannot continue to let someone else be in charge of our health. It is way too important. It is the time to take back our health.

While I understad the economy is not what it used to be, as always, an ounce of prevention is worth a pound of cure. Start exercising every day, stop eating flour and sugar, eat more fruits and vegetables, take a good quality supplement every day, take of your body now, before you end up in trouble down the road and have to rely on the government to fix it. Trust me, that is not going to work out too well. Spend a little time, money and energy getting your health back now, before its too late!
